Rogue-agent reporting shows AI labs need a formal investigation process

The most worrying part of a rogue-agent story is not that a model failed. It is the possibility that no formal process exists to investigate what happened, preserve evidence, and tell affected parties what changed afterward.

Agent systems create messy incidents because they combine model behavior, tools, permissions, memory, and human oversight. Without a repeatable process, every failure becomes a one-off argument about intent and responsibility.

AI labs should build incident response before agents become routine infrastructure. Customers will want audit trails, disclosure standards, and proof that the same behavior cannot quietly recur.
